Client Consent for eyelash extensions training course/class:
I have agreed to be trained by The Lux & Lash Co. to be taught eyelash extensions theory and application. Before my technician can preform this training course/class, I understand I must complete this agreement and provide my informed consent by signing and dating where indicated below.
For valuable consideration, in order to have The Lux & Lash Co. eyelash extensions applied and/or removed from my eyelashes.
In this class I understand that I will be taught The Lux & Lash Co’s (Natahlia Taborek) technique and understand that there are many techniques used in the industry and that this technique may not work for me. I understand that practice post the course is the only way to build skill and speed in the application of eyelash extensions and that I may not be able to perform the application perfectly on the day of the training course which is why The Lux & Lash Co offers 12 months on going support if needed. The Lux and Lash Co has designed a student manual (training handbook) for students to use at own desire. I understand that Lux and Lash co is not responsible for the failure of students understanding the training course’s materials or application. I agree that the Lux and Lash Co is not responsible for any issues to do with student to student application on live models for practicing technique of lash extensions at the training course. I understand that The Lux and Lash Co is not responsible for any issues that occur in my application of eyelash extensions or my career in the eyelash extensions industry after this course is complete as that is only in the control of myself (student) and waive The Lux and Lash Co from any involvement or responsibility after this course. I understand that The Lux and Lash Co only grants a non-accredited certificate of completion.

1. Waiver of liability
I understand that there are risks associated with having artificial eyelash extensions applied to and/or removed from my existing eyelashes, and that not withstanding the utmost of care in the application or removal of these products, there are still risks associated with the procedure and product itself, which include, without limitation, eye irritation, eye pain, discomfort, and in rare cases blindness when improperly handled. As part of this procedure I understand that certain amounts of eyelash adhesive material will be used to attach the artificial eyelash extension to the natural eyelash. Even though the Professional or The Lux & Lash Co. may teach apply or remove properly, I understand adhesive material may become dislodged during or after this procedure which may irritate my eyes or require further follow up care at my own expense to prevent damages to my eyes. I also understand that there is more than one technique of applying The Lux & Lash Co. eyelash extensions to my eyelashes and I will not attribute and liability to The Lux & Lash Co. Professional as a result of this procedure or the use and care of these lashes. I also agree to defend, indemnify and hold harmless Professional and The Lux & Lash Co. for any and all claims, actions, expenses, damages and liabilities, including reasonable attorney fees which might be asserted against as a result of having this procedure preformed, or my purchase of The Lux & Lash Co. products. As used in this agreement the terms ‘professional’ and ‘The Lux & Lash Co.’ include all of their respective officers, directors, agents, employees, technicians, beauticians, successors and assigns.
2. Permission to use pictures
I hereby grant to Professional and The Lux & Lash Co. the full right to take, publish, and reproduce photographs of me/my face/my eyes and/or eyelashes, both before and after this procedure, for any advertising, educating or any other purposes whatsoever, including the right to retouch these photographs as deemed necessary by Professional or The Lux & Lash Co. I further expressly assign any copyright in these photographs to The Lux & Lash Co. I also grant consent for Professional and The Lux and Lash Co. to use my pictures and likeness as contained in these photographs for any advertising purposes or other purposes along with any comments I may provide. Pictures will be used without a stated name of person.
3. Care and maintenance
I agree to follow the care and maintenance instructions provided by The Lux & Lash Co. and/or Professional for the use and care of my The Lux & Lash Co. lashes and that if any follow up care is required due to my own mistake or negligence, or failure to follow these instructions, this will be my own expense and risk. I understand that if I do any of the following it may result in damage to The Lux & Lash Co. lashes or may cause my lashes to fall off prematurely. Knowing this I agree to follow these tips for best results: I will avoid oil based products as these will loosen the bond of my Lashes by The Lux & Lash Co. I will avoid getting my lashes wet within the first 24-48 hours after my application. For the first two days after application I understand it is best to avoid swimming, saunas, or steam rooms. If I experience any itching or irritation, I agree to contact The Lux & Lash Co. Professional immediately to have the lash extensions removed. I agree to avoid using water proof mascara and to not use an eyelash curler, perm, or tint my lashes by The Lux & Lash Co. I agree to not pick, pull or rub my lashes by The Lux & Lash Co. I understand that I should not attempt to remove my lash extensions on my own or with any products, and to have them removed by a qualified eyelash extensions technician.
4. No known Medical Conditions/Informed consent
I have read and completed The Lux & Lash Co. consultation client consent form in its entirety and in truth. I acknowledge that I have been advised of the potential harmful or negative side effects (such as premature shedding of my eyelashes) that the lash extension procedure or removal may cause to those who have specific medical or skin conditions. I understand that the adhesives and adhesive remover are a skin, eye and mucus membrane irritant and that in rare cases persons may be allergic or have hypersensitivity to synthetics, cyanoacrylate or formaldehyde which in small amounts may be present in adhesives and removers. I understand that the procedure requires that I lay down still for up to 2 hours or longer with my eyes shut, and that if I wear contacts, I must remove my contact lenses for the duration of the lash extension application procedure or removal procedure. I further state that I have no known medical conditions that might be aggravated by the procedure or any medical conditions that would prevent me from complying with or heeding to the professional’s or The Lux & Lash Co.’s instructions or these warnings.
If any action is brought to enforce for this procedure, and all future procedures conducted by Professional or any other professional conducting business at the salon/home based salon establishment listed above.
I agree that this Agreement is binding upon me, and my heirs, legal representatives and assigns. I represent that I am over 18 years of age and that I have the right to enter into this agreement, or am under 18 years of age and have had my parent or legal guardian consent to this agreement.

I understand that there are many techniques and ways in which eyelash extensions can be applied and taught in to students and I waive The Lux & Lash Co. from liability to any unforeseen issues that may occur after and during training has finished. I understand that I will be taught a professional technique used by The Lux & Lash Co and this technique may not work for myself but knowing the fundamentals is highly important and I agree to abide by all safety precautions and maintain infection control standards for all my future clients. I will not attribute and liability to The Lux & Lash Co. Professional as a result of this class/training course or the use and care of the lashes applied during training by another student or my teacher (the professional). I also agree to defend, indemnify and hold harmless Professional and The Lux & Lash Co. for any and all claims, actions, expenses, damages and liabilities, including reasonable attorney fees which might be asserted against as a result of undergoing this class/training course, or my purchase of The Lux & Lash Co. products. As used in this agreement the terms ‘professional’ and ‘The Lux & Lash Co.’ include all of their respective officers, directors, agents, employees, technicians, beauticians, successors and assigns.
Signing this indicates that you have read, understood and agree with the Waiver, Client Consent and the Terms & Conditions at The Lux & Lash Co. which you have checked the box of, within this form.
